Southwark Clinical Commissioning Group (CCG) wrote to patients on 24 January 2018 to advise that the two GP contracts ran by The Hurley Group at the Lister Primary Care Centre were due to end on 30 September 2018. They advised that going forward a new single contract would be put in place for both of these practices and that the CCG would choose the provider by running a formal selection process.
We are pleased to inform that from 1 October 2018, AT Medics, led by Dr Abbasi, will be responsible for the care of patients registered with The Hurley at the Lister and the Caretaking Practice formally ran by Dr Hossain. Details of an upcoming patient meeting with our team can be found below.
The contract will last for 5 years and we will have the option to extend for another 5 years if patients and the CCG are happy with the service being provided. The new single practice will be called “The Lister Practice”. The phone number for The Lister Practice from 1 October 2018 will be 020 3049 8430.
AT Medics have worked in some of the most deprived areas in London, successfully increasing patient access to care and the service provision. We have also worked in difficult, poorly performing practices and have rapidly improved performance and efficiency, as well as developing and retaining a steady team. AT Medics will be focused on responding to and understanding the specific needs of patients at The Lister Practice whilst working to improve cooperation with local community organisations and engage better with patients on enhanced health promotion.
What does this mean for you as a patient?
If you are happy with these arrangements, you do not need to do anything. There will be no changes to your registration, but please note the single telephone number for the practice from 1 October 2018 will be 020 3049 8430.
Your medical records will remain at the practice and you will be able to receive services as usual from the surgery. If at any time you wish to change your GP practice, you can find information about other local practices on the NHS website.

On 8 October 2018 at 6pm we will be holding a meeting at the practice for patients to meet some of the new team. You will also have the opportunity to ask questions and learn more about the services that will be offered and how you can become involved in shaping future services provided from your surgery.
